ROUTINE APPOINTMENTS AND EXTENDED HOURS

Routine appointment scheduling, general information, and prescription refills will be handled during routine office hours. Same day, early morning, evening and weekend appointments are available. Simply call us to arrange an appointment time that is convenient for you!

 

Messages taken by the schedulers will be given to our “Patient-Centered Medical Home” team who will either call you or relay the information to the physician or physician’s assistant.

 

When calling for an appointment, please indicate to the receptionist your symptoms and the nature of the consultation requested. This enables us to reserve an adequate amount of time for your particular health care needs. New patients are asked to arrive for their appointment 20 minutes early. We will take a copy of your insurance card and verify your insurance as you wait.

 

A sincere attempt is made to adhere to our appointment schedule. Because emergencies do arise and will complicate the best of schedules, we ask for your understanding at these times. If you will be late for, or cannot keep an appointment, please give us 24 hours notice.

 

FOR EVERY APPOINTMENT PLEASE BRING:
  • Your insurance card and photo identification.
  • A list of current prescription and non-prescription medications, vitamins and supplements.
  • A description of the problem, how long you have had it and how it affects you.
  • A list of questions you would like to discuss with a member of the team.

 

FINACIAL PROCEDURES
 

You are required to give us a copy of your insurance card on your first visit and each time thereafter. For your convenience, we bill all insurance companies including Medicare. Please be prepared to pay your co-payment (if applicable) and any outstanding patient balance due when you check in for your appointment. We accept checks, cash, American Express, MasterCard, and Visa.

AFTER-HOURS COVERAGE
Call (734) 692-2337 if you would like to speak to a clinician to help you decide how to treat an illness after hours or to help you decide whether to go to the emergency room or the nearest urgent care. This is an after hours service we offer. If you receive care at an emergency room or urgent care center, please let us know by calling (734) 285-0677 within 48 hours so we can assist with follow up care as needed.
